MOORPARK AYSO FALL 2010 REGISTRATION
For Players and Volunteers
Registration Information
Open To:
Boys and Girls, ages 4 to 18 years old (birth dates between August 1st, 1991 and July 31, 2006)
No previous soccer experience is needed. Boys and girls play on separate teams.
*A Birth Certificate is required if your child is a first time player in Moorpark AYSO*
Registration Fees:
Registration begins April 1st through June 13th. The registration fees include uniform, pictures, supplemental insurance and 11 weeks of soccer.
"Early-Bird Discount" - Begins April 1st through May 23rd.
$90 for 1st player, $80 for 2nd player and $70 each additional player in the same family.
Regular Fees - After May 23rd.
The registration fee is a $100 per player. In addition, registrations received after June 13th will be placed on a wait list in order to maintain balanced teams.
Volunteers (Coach, Asst Coach, Team Parent, Referee, etc.) must also complete a volunteer application form every year.

Division
Two year age spans (two grade levels) make up a division. Your child's birthdate determines which division she/he will be playing in.

Registration Process
There are two ways to register. You may register online or during one of our walk-in registrations days.
ONLINE REGISTRATION
To register online, please click the logo below.
Once logged into eAYSO...
- To register your child(ren), click on the AYSO parents button.
- To register as a parent volunteer, click on the AYSO volunteer button.
Once registered, please print out two copies of the completed Player Registration form.
If you plan on volunteering, please register and print out two completed Volunteer forms. You will also need to submit a copy of your CA Driver's License.
Mail signed forms, fees, proof of birth date (if first time player) and CA Driver's License (if volunteering) to:

WALK-IN REGISTRATION
at Arroyo Vista Recreation Center - 4550 Tierra Rejada Rd
Saturday .......... April 24th .......... 9:00 am to 4:00 pm
Saturday .......... May 22nd ........... 9:00 am to 4:00 pm
Teams
Coaches will receive their team information late July. Your coach will contact you within the first two weeks of August. If you haven't received a call by August 10th, please follow up with the Division Director responsible for your child's age group. If you registered late and your child was placed on a waiting list, we will contact you regarding your child's team assignment.
Refund Policy
Requests for refunds will be accepted through July 1st. Please submit your refund request to refunds@moorparkayso.org. In your request, please include the following:
- Child's Full Name
- Amount Paid
- Payment Type (cash or check). If paid by check, please include check #.
AFTER July 1st, there will be no refunds.
